This is our form. The process is, the student leaves with parent or EMT, I am to have original inter-district mail. A head’s up email is always welcome. If they return to class, the form is kept at the site. This is during the course of the school day, the athletics should have an entirely different procedure. This was for me as a head’s up for risk exposure. Hope this helps.

Hello,
I don’t do in-person training anymore. There is a video on our training module. And then, we have a hard copy acknowledgement form that is completed and put in a file.

When these things cross my desk, the first thing I do is go the Risk Pool exclusion or high-risk activities. That will answer how to proceed. If it is high-risk, then the process continues like any other club protocol. I am not the ultimate decision-maker, it would up the chain from there. Hope this helps.
