Volunteer Automobile Use Forms

I am reaching out to see if anyone has had major push back with volunteer transportation forms? We have a shortage of bus drivers, so parents are driving students to extra-curricular events. We ask them to fill out a Volunteer Auto Use form that requires them to provide insurance declarations papers to see how much $ coverage they carry. Also, they have to fill it out even if they are only driving their own child. What is everyone else doing out there?
