We are looking at revising our Student Injury Forms. Can you please share what you are using? Thank you, and happy Friday!
This is our form. The process is, the student leaves with parent or EMT, I am to have original inter-district mail. A head’s up email is always welcome. If they return to class, the form is kept at the site. This is during the course of the school day, the athletics should have an entirely different procedure. This was for me as a head’s up for risk exposure. Hope this helps.
