Any districts fined for COVID violations?

I am putting together a presentation for the annual CASBO conference regarding COVID inspections (both internal and external). Has ANYONE heard of a district being inspected/fined/sued by any regulatory agencies like CalOSHA for violating COVID protocols? I know San Francisco sued its own district for NOT reopening, and teacher’s unions have sued districts for REOPENING, and there are some parent groups suing districts for not reopening too. Any information would be great and would enhance our presentation.
